12/2/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Josiah upon arrival to the home setting. Josiah began the session with a positive attitude. Josiah was smiling and more expressive. Josiah was in a positive mood throughout the entire session while is mother was discussing with QMHP-C on how he continues to struggle with communication skills. 2. Behavior: Josiah was withdrawn at times within the session when discussing with him as well as him mother ways to decrease his problematic behaviors within the home and community setting. 1.Intervention: QMHP-C engaged in self-controlling skills as the mother reported that it's been difficult to deal with Josiah’s behaviors on a daily basis. QMHP-C had Josiah to identify problem causing behaviors and inquired about his behaviors toward his mother. QMHP-C engaged Josiah in handout on anger control and learning to build positive communication skills through healthy expressions. 2. Intervention: QMHP-C had Josiah list positive ways to communicate his thoughts rather than act out the behaviors. QMHP-C modeled effective ways to verbalize rather than act out feelings and engaging on negative decision-making. QMHP-C role-played and demonstrated several emotions with Josiah to give insight into his behaviors. QMHP-C discussed taking responsibility for your own actions and learning to say "I" first before blaming others. 1. Response: Josiah and family was receptive to QMHP-C's suggestions for today's session. Josiah was cooperative with QMHP-C in revealing his behaviors toward his mother. Josiah reported that he was frustrated with his mother because he used profanity towards him for doing something wrong. Josiah was frustrated and indicated that he held his frustrations and verbalized that if his mother did not want him to use that language then neither should he. 2. Response: QMHP-C gave encouragement to Josiah for being the bigger person and not acting out disrespectfully. Josiah indicated that his mother make it difficult to communicate when he's upset.
